Natural 20 Table — Perception Skill Checks, d20

A free d20 table for a natural 20 on a Perception check in 5e. Twenty outcomes, from solid to legendary.

A natural 20 on a skill check is not a critical success in fifth edition — criticals apply to attack rolls only, and a 20 on a check is simply a 20. Which is why a legendary roll so often feels like nothing happened. This table gives it somewhere to land.

Twenty results written for Perception specifically. A legendary Perception roll has nothing in common with a legendary roll at anything else, which is why there are eighteen of these and not one.

Roll d20 when a player rolls a natural 20 on a skill check and succeeding quietly would be a waste.

Severity tiers

Each result names its tier, so you can see at a glance how bad a roll was without decoding a colour.

The table

20 results. Roll d20.

d20Result TierWhat happens
1 Saw It Solid You spot exactly what was hidden. No extra effect.
2 And the Second One Solid There was more than one thing concealed here. You found both.
3 Before It Moved Solid You notice in time to act. The party is not surprised.
4 Counted Them Solid You know exactly how many are out there and where each one is.
5 Heard the Words Solid You catch what is being said out of earshot, clearly enough to repeat it.
6 The Draught Solid You notice airflow, damp, or sound that reveals a space nobody knew was there.
7 Sharp Solid You have advantage on your next Perception check this scene.
8 Called It Out Solid One ally has advantage on their next Perception check here.
9 Watched the Watcher Solid You realise you are being observed, and by whom.
10 The Odd Detail Solid Something here is subtly wrong. The DM tells you what, and it matters later.
11 Saw Through It Impressive You see an illusion, disguise, or glamour for what it is, without a save.
12 First Move Impressive You act before the ambush lands. You have advantage on initiative and act first regardless of the roll.
13 The Whole Field Impressive One check covers the entire area. Nothing here is concealed from you.
14 Named the Threat Impressive You identify not just that something is there but exactly what it is, and one of its capabilities.
15 Warned Everyone Impressive Every ally who can hear you shares the benefit — nobody is surprised, and all have advantage on initiative.
16 Read the Approach Impressive You know how they intend to come at you, and can position the party accordingly.
17 Never Lost Them Impressive You can track this creature by sight or sound for the rest of the scene, however it tries to break away.
18 Nothing Escapes You Legendary For the rest of this scene you automatically notice any hidden creature, object, or effect. No rolls.
19 The Thing Nobody Saw Legendary You notice something the DM had not planned for the party to find yet. They should let you have it anyway.
20 The One They'll Talk About Legendary You spot the one detail that unravels the whole situation — the impostor, the trap, the real threat. The scene changes shape because of it.

Questions

Is a natural 20 a critical success on a skill check?

Not by the rules. Criticals apply to attack rolls. A 20 on a check is a 20 — which is why a spectacular roll so often produces a shrug. This table is the house rule that fixes that.

Does a natural 20 automatically succeed?

No. On an ability check there is no automatic success — if the DC is 25 and your total is 22, you failed with a 20.
