Critical Fumble Table — Ranged Attacks (Monsters), d100

A free d100 critical fumble table for ranged attacks in 5e — 100 hand-written outcomes, graded by severity.

Fifth edition has no critical fumble rule. A natural 1 on an attack roll simply misses — that is all the rules say. Everything below is an optional table for tables that want a miss to be worth describing, and it is the DM's call whether to use one at all.

The usual method: when a player rolls a natural 1 on an attack roll, roll d100 here and read the result. Some tables only roll on a confirmed fumble — a second attack roll that also misses — which makes the spectacular results roughly five times rarer. Severity climbs with the roll, so a low number is embarrassing and a high one is expensive.

These hundred results are written specifically for ranged attacks made by monsters, so the failure fits what was actually being attempted — a fumbled spell goes wrong differently from a dropped axe.

8 Bent Shaft Clumsy That piece of ammunition is spent and can't be recovered afterwards.
9 Wrong Missile Clumsy It threw entirely the wrong thing — a rock instead of a javelin, a rations tin instead of a spine. No effect.
10 Overdrawn Clumsy It has disadvantage on its next Strength check.
11 Snapped Nock Clumsy One piece of ammunition splits and is ruined.
12 Shot the Scenery Clumsy The missile buries itself somewhere conspicuous and stays there for the rest of the fight.
13 The Wind Took It Clumsy No mechanical effect. The creature glares at the sky as though it were personal.
14 Cramped Grip Clumsy Its next attack this turn, if it has one, deals 1 less damage (minimum 1).
15 Twisted Sling Clumsy Its sling, strap, or launching membrane fouls. Righting it costs a free object interaction.
16 Misjudged the Range Clumsy No penalty — but tell the players how badly the shot fell short. They'll adjust their positioning.
17 Frustrated Screech Clumsy It gives its exact position away to everything that can hear, which may include things it would rather not attract.
18 Slipped Stance Clumsy It can't take a bonus action this turn if it hasn't already used one.
19 Grit in the Works Clumsy If its weapon has the loading property, its next reload takes noticeably longer. Otherwise cosmetic.
20 It Ranges In Clumsy The miss tells it exactly how far off it was. No penalty, and it has advantage on its next attack roll against that target.
21 Fouled String Setback Its next attack roll this turn is made with disadvantage.
22 Ammunition Spilled Setback 1d4 pieces of its ammunition scatter into its space.
23 Bad Sightline Setback Its target has three-quarters cover against it until the creature moves to a different space.
24 Position Revealed Setback It grants advantage to the next attack roll made against it before the start of its next turn.
25 Slow Draw Setback It can't take reactions until the start of its next turn.
26 Shot into Cover Setback The missile thuds into an allied creature's shield. That ally's next attack roll is made with disadvantage.
27 Overreached Setback Its speed is reduced by 10 feet until the end of its next turn.
28 Cramped Shoulder Setback It has disadvantage on the next Strength check or Strength saving throw it makes.
29 Jammed Reload Setback If its weapon has the loading property, it can't fire until it uses an action to clear the jam.
30 Frayed String Setback Its next attack deals 2 less damage (minimum 1).
31 Startled by the Snap Setback If it's concentrating on a spell or effect, it must succeed on a DC 10 Constitution saving throw or lose concentration.
32 Off-Rhythm Setback On its next turn it can move or take an action, not both. Bonus actions still work.
33 Wasted Volley Setback It can't make any additional attacks this turn, even if it had them remaining.
34 Dazzled Setback It has disadvantage on its next attack roll and its next Perception check.
35 Read by the Party Setback The next creature to make a saving throw against one of its abilities has advantage on that save.
36 String Welt Setback The string catches it across the limb. It takes 1 damage.
37 Quiver Askew Setback It must spend 5 feet of movement righting its ammunition, or draw at disadvantage on its next turn.
38 Supply Doubt Setback The DM should start tracking this creature's ammunition honestly from here. It has fewer shots left than it was behaving as though it had.
39 Too Close to a Friend Setback An allied creature within 5 feet of its target flinches. That ally's next attack roll is made with disadvantage.
40 Braced Wrong Setback It isn't prone, but it's set awkwardly. Standing fully costs 5 feet of movement.
41 Weapon Dropped Blunder Its weapon falls into its space. Picking it up costs an object interaction.
42 Weapon Flung Blunder The weapon skitters 10 feet away in a random direction.
43 Bowstring Snaps Blunder The weapon is unusable until it spends an action restringing. If it has no spare, the weapon is useless for the rest of the encounter.
44 Turn Wasted Blunder The whole motion falls apart. Its turn ends immediately.
45 Shot an Ally Blunder An allied creature within 30 feet of its target takes one roll of its weapon's damage die, with no modifiers.
46 Ammunition Scattered Blunder Half its remaining ammunition for this weapon (round down) spills 10 feet away.
47 Rooted Blunder Its speed is 0 until the end of its next turn.
48 Concentration Broken Blunder It loses concentration on any spell or effect it's maintaining. No saving throw.
49 Pinned to Cover Blunder Its own shot pins hide, cloak, or wing to the cover beside it. Working free takes an action, or 1 damage tearing loose.
50 Fell Back Blunder The recoil overbalances it. It falls prone.
51 Snapped Back Blunder String or limb whips into it. It takes one roll of its weapon's damage die, with no modifiers.
52 Fouled Sight Blunder It's blinded until it uses an object interaction to clear its eyes.
53 Gear Cut Loose Blunder A piece of its visible gear tears free and falls into its space.
54 Wild Loose Blunder The creature nearest to it — ally or enemy — must succeed on a DC 10 Dexterity saving throw or take 1 damage.
55 Out of Cover Blunder It lurches into the open and loses the benefit of any cover until it moves to a new space.
56 Shaky Aim Blunder It has disadvantage on all attack rolls until the end of its next turn.
57 Quiver Torn Away Blunder Its ammunition container is ripped loose and lands 10 feet away.
58 Backpedal Blunder It stumbles 10 feet directly away from its target.
59 Cracked Limb Blunder The weapon deals 2 less damage (minimum 1) for the rest of the encounter.
60 Crashing Fall Blunder It falls prone and drops whatever it was holding.
61 String Lash Disaster It takes damage equal to one roll of its weapon's damage die plus its ability modifier.
62 Torn Shoulder Disaster It has disadvantage on all attack rolls for the rest of the encounter.
63 Through a Friend Disaster The allied creature nearest its target takes one roll of its weapon's damage die plus its ability modifier.

68 Footing Collapsed Disaster The ground gives beneath it. It falls prone and is restrained until it uses an action to climb out.

70 Marked Disaster The creature it missed has advantage on attack rolls against it until the end of that creature's next turn.
71 Weapon Broken Disaster A non-magical ranged weapon is ruined. A magical one is warped and deals 2 less damage (minimum 1) for the rest of the encounter.
72 Position Blown Disaster Every hostile creature that can see it now knows exactly where it is. It loses the benefit of cover until it relocates.
73 Ribs Cracked Disaster It has disadvantage on Constitution saving throws for the rest of the encounter.
74 Pinned an Ally Disaster Its shot pins an allied creature to cover. That ally is restrained until it or another creature uses an action to free it.
75 Snuffed Its Own Light Disaster Any light source the creature carries or produces goes out.
76 Winded and Down Disaster It falls prone, and its speed is 0 until the end of its next turn.
77 Dropped Its Prize Disaster Something the creature was carrying — DM's choice, and it should be something the party wants — falls into its space.
78 Numb Limbs Disaster It drops everything it's holding and can't pick anything up until the end of its next turn.
79 Deafened by the Crack Disaster It's deafened for 1 minute and can no longer hear its allies.
80 Recoil Disaster It takes one roll of its weapon's damage die and falls prone.
81 Ally Struck Hard Catastrophe The allied creature nearest its target takes the attack's full damage, modifiers included, exactly as though it had been hit.
82 Weapon Shattered Catastrophe A non-magical weapon is destroyed outright. A magical one is unusable for the rest of the encounter.
83 Shaft Through Its Hand Catastrophe It takes double its weapon's damage die and is badly wounded: disadvantage on attack rolls until it regains any hit points.
84 Thrown Clear Catastrophe It's hurled 15 feet in a random direction, lands prone, and takes 2d6 bludgeoning damage.
85 Blinded Catastrophe It's blinded for the rest of the encounter.
86 Drawing Limb Broken Catastrophe The limb is ruined — permanently, barring magical healing. It loses any attack that requires two limbs, which for most archers means all of them.
87 Structural Collapse Catastrophe Its shot brings something down. Every creature within 10 feet must succeed on a DC 13 Dexterity saving throw or take 2d6 bludgeoning damage, and the DM may destroy a piece of cover permanently.
88 Roused the Wrong Thing Catastrophe The noise carries far too well. Something arrives in 1d4 rounds, and it is not necessarily on the monster's side.
89 Prisoner Freed Catastrophe Anything the creature was grappling, restraining, or otherwise holding is released immediately.
90 Ammunition Exhausted Catastrophe All its remaining ammunition for this weapon is destroyed or scattered beyond reach. It is down to whatever it can do in melee.
91 Weapon in the Party's Hands Catastrophe The weapon lands in a player character's space. They may pick it up as a free object interaction.
92 Concussed Catastrophe It's stunned until the end of its next turn, and has disadvantage on all ability checks for the rest of the encounter.
93 Friendly Fire, Badly Catastrophe Make the attack roll again against the allied creature nearest its target, using the same modifiers. On a hit, that ally takes full damage.
94 Everything Falls Catastrophe It drops its weapon, its ammunition, and one carried item, each into a different space within 10 feet.
95 Out of the Fight Catastrophe It falls prone, drops its weapon, and its turn ends. It can't take reactions until the start of its next turn.
96 The Shot Heard Everywhere Legendary Its shot severs something holding the place together — a chain, a support, a dam. The DM introduces a hazard or a countdown. The creature takes 4d6 bludgeoning damage and is prone and restrained under the debris until it uses an action to dig out.
97 Weapon Destroyed Utterly Legendary The weapon is gone — split to kindling, over the cliff, or gone into the water. A magical weapon does not survive this either, and the party will never get to loot it.
98 Struck Its Own Champion Legendary The attack resolves in full against the most dangerous allied creature in range, as a critical hit. Roll the damage dice twice.
99 Knocked Itself Out Legendary The creature is unconscious for 1 minute, or until it takes damage. For most encounters that simply ends its participation, and the party should be encouraged to capitalise.
100 The One They'll Tell Stories About Legendary It ends up disarmed and prone with its own ammunition lodged somewhere absurd and highly visible. Every creature that can see it has advantage on attack rolls against it until the start of its next turn, and each of its allies that witnessed this must succeed on a DC 10 Wisdom saving throw or have disadvantage on its next attack roll.

Questions

Do critical fumbles exist in 5e?

No. A natural 1 on an attack roll misses, and that is the whole rule. Fumble tables are a house rule, and a popular one — but nothing in the game requires them.

Should a natural 1 always miss?

Yes, on an attack roll — that part IS a rule. A table like this describes the miss; it does not change whether you hit.

Doesn't this punish martial characters?

It can. A fighter with three attacks rolls far more d20s than a wizard, so a per-roll fumble rule falls hardest on the people who attack most. Rolling only on a confirmed fumble evens it out considerably.
