Critical Fumble Table — Spell Attacks (Players), d100

A free d100 critical fumble table for spell attacks in 5e — 100 hand-written outcomes, graded by severity.

Fifth edition has no critical fumble rule. A natural 1 on an attack roll simply misses — that is all the rules say. Everything below is an optional table for tables that want a miss to be worth describing, and it is the DM's call whether to use one at all.

The usual method: when a player rolls a natural 1 on an attack roll, roll d100 here and read the result. Some tables only roll on a confirmed fumble — a second attack roll that also misses — which makes the spectacular results roughly five times rarer. Severity climbs with the roll, so a low number is embarrassing and a high one is expensive.

These hundred results are written specifically for spell attacks made by players, so the failure fits what was actually being attempted — a fumbled spell goes wrong differently from a dropped axe.

7 Cold Fingers Clumsy Your hands go briefly numb. No effect — you flex them and carry on.
8 Afterimage Clumsy Coloured spots swim across your vision. No mechanical effect, but it's distracting.
9 Wrong Word Clumsy The spell fails and you finish the sentence in a language you don't remember learning. Harmless.
10 Focus Slipped Clumsy Your arcane focus or holy symbol twists in your grip. Righting it costs a free object interaction.
11 Ears Popped Clumsy You have disadvantage on your next Perception check that relies on hearing.
12 Sparks Clumsy Small sparks scorch your sleeve. Cosmetic, and slightly embarrassing.
13 Feedback Hum Clumsy A single sustained note only you can hear. No effect, and mildly maddening.
14 Chalk Smudged Clumsy If your casting involves drawn or written components, redrawing costs a free object interaction.
15 Muscle Memory Failed Clumsy No effect. The gesture was simply, unmistakably wrong, and you know it.
16 Momentary Vertigo Clumsy You can't take a bonus action this turn if you haven't already used it.
17 Cold Snap Clumsy Frost blooms across nearby surfaces. Purely cosmetic, and rather beautiful.
18 Familiar Startled Clumsy If you have a familiar, it bolts and won't come near you until the end of your next turn.
19 Arcane Hiccup Clumsy No mechanical effect now. It will recur at the least convenient moment, at the DM's discretion.
20 Instructive Failure Clumsy You feel exactly where the weave slipped. No penalty, and the next creature to make a saving throw against one of your spells before the end of your next turn does so with disadvantage.
21 Weave Slips Setback Your next attack roll this turn is made with disadvantage.
22 Pouch Spilled Setback 1d4 non-costly material components fall into your space.
23 Somatic Cramp Setback You have disadvantage on the next Dexterity saving throw you make.
24 Exposed in the Casting Setback You grant advantage to the next attack roll made against you before the start of your next turn.
25 Slow Recovery Setback You can't take reactions until the start of your next turn.
26 Feedback Jolt Setback If you're concentrating on a spell, make a DC 10 Constitution saving throw or lose concentration.
27 Rooted by Backlash Setback Your speed is reduced by 10 feet until the end of your next turn.
28 Voice Lost Setback You can't cast spells with verbal components until the end of your next turn.
29 Focus Fouled Setback Your next spell attack deals 2 less damage (minimum 1).
30 Mana Burn Setback The backlash nips at you. You take 1 damage.
31 Wild Flicker Setback A harmless illusory echo of the spell blooms 10 feet away. One creature of the DM's choice turns to look at it.
32 Off-Rhythm Setback On your next turn you can move or take an action, not both. Bonus actions still work.
33 Spent Momentum Setback You can't cast another spell this turn, not even as a bonus action.
34 Dazzled by Your Own Light Setback You have disadvantage on your next attack roll and your next Perception check.
35 Telegraphed Casting Setback The next creature to make a saving throw against one of your spells before the end of your next turn has advantage on it.
36 Nosebleed Setback You take 1 damage and look distinctly unwell for the rest of the encounter.
37 Ringing in the Weave Setback You have disadvantage on the next Intelligence check you make.
38 Component Miscount Setback You no longer know what's left in your pouch. The DM tracks your non-costly components secretly until your next short rest.
39 Backdraft Setback An ally within 5 feet catches the edge of it. Their next attack roll is made with disadvantage.
40 Knelt in the Circle Setback You aren't prone, but you're braced low and awkwardly. Standing fully costs 5 feet of movement.
41 Focus Dropped Blunder Your arcane focus or holy symbol falls into your space. Picking it up costs an object interaction.
42 Focus Flung Blunder Your focus skitters 10 feet away in a random direction.
43 Slot Guttered Blunder The spell slot is expended with nothing to show for it. If this was a cantrip, you can't cast that cantrip again until the end of your next turn.
44 Turn Unravels Blunder The whole casting comes apart in your hands. Your turn ends immediately.
45 Backlash on an Ally Blunder An ally within 5 feet takes 1d6 force damage.
46 Components Scattered Blunder Half your non-costly material components (round down) spill 10 feet away.
47 Knocked Flat Blunder The recoil puts you on the floor. You fall prone.
48 Concentration Broken Blunder You lose concentration on any spell you're maintaining. No saving throw.
49 Grasping Weave Blunder Half-formed spectral hands catch your legs. You're restrained until you use an action to break free with a DC 12 Arcana or Athletics check.
50 Rooted Blunder Your speed is 0 until the end of your next turn.
51 Feedback Burn Blunder You take 1d6 force damage.
52 Blinding Flare Blunder You're blinded until the end of your next turn.
53 Symbol Scorched Blunder A piece of your visible gear is singed loose and falls into your space.
54 Wild Discharge Blunder The creature nearest to you — ally or enemy — must succeed on a DC 10 Dexterity saving throw or take 1d4 force damage.
55 Beacon Blunder Every hostile creature within 60 feet now knows there is a spellcaster here, and precisely where you are standing.
56 Trembling Hands Blunder You have disadvantage on all attack rolls until the end of your next turn.
57 Pouch Torn Away Blunder Your component pouch is ripped loose and lands 10 feet away.
58 Recoil Step Blunder You're shoved 10 feet directly away from your target.
59 Focus Cracked Blunder Your spell attacks deal 2 less damage (minimum 1) until the focus is mended over a short rest.
60 Collapse Blunder You fall prone and drop whatever you were holding.
61 Arcane Burn Disaster The backlash goes through you. You take 2d6 force damage.

63 Backlash Through a Friend Disaster The ally nearest to you takes 2d6 force damage.

65 Focus Lost to the Dark Disaster Your focus is flung 20 feet in a random direction, into whatever the terrain offers.
66 Stunned by Feedback Disaster You're stunned until the end of your next turn.
67 Wards Unravelled Disaster Every spell you currently have active with a duration ends immediately.
68 Ground Warped Disaster The floor buckles beneath you. You fall prone and are restrained until you use an action to pull free.
69 Blinded by Backlash Disaster You're blinded until the end of your next turn.
70 Attention of the Weave Disaster The creature you targeted has advantage on attack rolls against you until the end of its next turn.
71 Focus Broken Disaster A non-magical focus is ruined. A magical one is merely dimmed — your spell attacks deal 2 less damage (minimum 1) until it's restored over a short rest.
72 Drawn In Disaster You're hauled 10 feet toward your target and provoke an opportunity attack from every hostile creature that can reach you.
73 Frayed Nerves Disaster You have disadvantage on Constitution saving throws until you finish a long rest.
74 Backwash Wave Disaster Every ally within 5 feet must succeed on a DC 12 Dexterity saving throw or fall prone.
75 Light Snuffed Disaster Every non-magical light source within 10 feet goes out, and any magical light you created ends.
76 Winded and Down Disaster You fall prone, and your speed is 0 until the end of your next turn.
77 Pouch Emptied Disaster One item from your inventory — DM's choice, never an attuned one — falls into your space and may be trampled.
78 Numb to the Weave Disaster You can't cast any spell until the end of your next turn.
79 Deafened by the Discharge Disaster You're deafened for 1 minute.
80 Recoil Disaster You take 1d6 force damage and fall prone.
81 Ally Caught in Full Catastrophe The ally nearest to you suffers the spell's full effect as though they had been the target. If it allows a saving throw, they make it.
82 Focus Shattered Catastrophe A non-magical focus is destroyed outright. A magical one survives but is unusable until an hour's work restores it.
83 Turned Inward Catastrophe You suffer the spell's own effect as though you had been the target. If it allows a saving throw, you make it.
84 Thrown Clear Catastrophe You're hurled 15 feet in a random direction, land prone, and take 1d6 force damage.
85 Sightless Catastrophe You're blinded until you regain any hit points or an ally spends an action tending to you.
86 Casting Hand Seized Catastrophe Your hand locks rigid. You can't cast spells with somatic components until a lesser restoration or a long rest with proper care.
87 Structural Collapse Catastrophe The discharge brings something down. Every creature within 5 feet of you, yourself included, must succeed on a DC 13 Dexterity saving throw or take 2d6 bludgeoning damage.
88 Something Noticed Catastrophe The surge is felt somewhere it shouldn't be. A wandering monster, an extraplanar curiosity, or a reinforcement of the DM's choice arrives in 1d4 rounds.
89 Bindings Undone Catastrophe If the target was grappled, restrained, charmed, frightened, or otherwise controlled by you or an ally, that control ends immediately.
90 Slots Seared Catastrophe You lose your two lowest-level remaining spell slots.
91 Focus in Enemy Hands Catastrophe Your focus lands in a hostile creature's space. It may pick the focus up as a free object interaction.
92 Concussed by Feedback Catastrophe You're stunned until the end of your next turn, and you have disadvantage on Intelligence, Wisdom, and Charisma checks until you finish a long rest.
93 Backlash, Badly Catastrophe The spell resolves in full against the ally nearest to you. If it called for an attack roll, make that roll against them; if it allows a save, they make it.
94 Everything Falls Catastrophe You drop your focus, your component pouch, and one other held item, each into a different space within 10 feet.
95 Out of the Fight Catastrophe You fall prone, drop your focus, and your turn ends. You can't take reactions until the start of your next turn.
96 The Weave Tears Legendary A rift opens where the spell should have been. The DM introduces a hazard, a countdown, or something on its way through. You take 2d6 force damage and fall prone.
97 Pulled Out of the World Legendary Your focus is drawn somewhere else entirely — through the rift, into the river, into a pocket of somewhere that isn't here. Recovering it is a side quest, not an action. A magical focus is always recoverable eventually; the DM decides how far you'll go.
98 Cast Upon a Friend Legendary The spell resolves in full against the ally nearest to you, treated as a critical hit if it deals damage — roll the damage dice twice. It worked flawlessly. It worked on entirely the wrong person.
99 Spectacular Self-Defeat Legendary The backlash puts you down. You're unconscious for 1 minute, or until you take damage or an ally uses an action to rouse you. You are not dying — you are simply overloaded.
100 The One They'll Tell Stories About Legendary The spell fails in the most conspicuous way magic can: wrong colour, wrong size, wrong direction, and audible three rooms away. You're prone and separated from your focus. Every creature that can see you, friend and foe alike, has advantage on attack rolls against you until the start of your next turn. You take no damage. Your standing as a professional takes all of it.

Questions

Do critical fumbles exist in 5e?

No. A natural 1 on an attack roll misses, and that is the whole rule. Fumble tables are a house rule, and a popular one — but nothing in the game requires them.

Should a natural 1 always miss?

Yes, on an attack roll — that part IS a rule. A table like this describes the miss; it does not change whether you hit.

Doesn't this punish martial characters?

It can. A fighter with three attacks rolls far more d20s than a wizard, so a per-roll fumble rule falls hardest on the people who attack most. Rolling only on a confirmed fumble evens it out considerably.
