Critical Fumble Table — Melee Attacks (Monsters), d100
A free d100 critical fumble table for melee attacks in 5e — 100 hand-written outcomes, graded by severity.
Fifth edition has no critical fumble rule. A natural 1 on an attack roll simply misses — that is all the rules say. Everything below is an optional table for tables that want a miss to be worth describing, and it is the DM's call whether to use one at all.
The usual method: when a player rolls a natural 1 on an attack roll, roll d100 here and read the result. Some tables only roll on a confirmed fumble — a second attack roll that also misses — which makes the spectacular results roughly five times rarer. Severity climbs with the roll, so a low number is embarrassing and a high one is expensive.
These hundred results are written specifically for melee attacks made by monsters, so the failure fits what was actually being attempted — a fumbled spell goes wrong differently from a dropped axe.
Severity tiers
Each result names its tier, so you can see at a glance how bad a roll was without decoding a colour.
- Clumsy 1–20 — Embarrassing. Costs you nothing but face.
- Setback 21–40 — A real but minor tactical cost.
- Blunder 41–60 — Costs you the turn, the weapon, or the position.
- Disaster 61–80 — You are now worse off than before you swung.
- Catastrophe 81–95 — Real loss. Someone is going to remember this.
- Legendary Fumble 96–100 — Rare, spectacular, and survivable. Describe it in loving detail.
The table
100 results. Roll d100.
| d100 | Result | Tier | What happens |
|---|---|---|---|
Wild Swing | Clumsy | The blow carves through empty air and the creature turns half around with it. No mechanical effect — describe the snarl.
Snapped Claw | Clumsy | A claw, tusk, or tooth chips off. Cosmetic, but the creature notices, and it does not like it.
Overreach | Clumsy | It commits far too much to the swing. No mechanical effect beyond looking foolish.
Ringing Impact | Clumsy | Its weapon rings off stone. Any creature within 60 feet that can hear is alerted to the fight.
Bit Its Own Tongue | Clumsy | No effect. It is now angry about something entirely unrelated to the party.
Skidded | Clumsy | It slides on loose ground and recovers. Purely cosmetic.
Roar of Effort | Clumsy | The sound gives its exact position away to anything that can hear, including things elsewhere in the dungeon.
Dust in Its Eyes | Clumsy | It has disadvantage on its next Perception check.
Fouled by Its Own Hide | Clumsy | Cloak, mane, or loose skin tangles the limb. The first 5 feet it moves on its next turn costs double.
Chipped Edge | Clumsy | A nick in the blade or claw. Cosmetic, and repairable if the creature is the sort that repairs things.
Wrong Grip | Clumsy | Its next attack this turn, if it has one, deals 1 less damage (minimum 1).
Struck the Wall | Clumsy | A chunk of masonry comes loose and clatters down. Atmospheric rather than dangerous.
Cracked Knuckle | Clumsy | It hits something hard and unyielding that isn't a player character. It hurts. It does nothing.
Snarl of Frustration | Clumsy | No mechanical effect, but its allies can hear the frustration and morale visibly dips.
Pulled Muscle | Clumsy | It has disadvantage on its next Strength check.
Slipped Harness | Clumsy | A strap twists across the creature. Righting it costs a free object interaction.
Telegraphed Badly | Clumsy | No penalty, but the players saw exactly what it was trying to do. Tell them.
Off-Balance Recovery | Clumsy | It can't take a bonus action this turn if it hasn't already used one.
Momentary Stumble | Clumsy | It catches itself on something. Cosmetic — but describe what it grabbed.
It Learns | Clumsy | The miss teaches it something about how this target fights. No penalty, and it has advantage on its next attack roll against that target. The mildest result on the table is the one that bites.
Overextended | Setback | It grants advantage to the next attack roll made against it before the start of its next turn.
Weapon Wide | Setback | Its next attack roll this turn is made with disadvantage.
Guard Dropped | Setback | Its AC is reduced by 2 until the start of its next turn.
Snagged | Setback | It must spend 5 feet of movement working itself free of whatever it caught on.
Wrong Foot | Setback | Its speed is reduced by 10 feet until the end of its next turn.
Reaction Spent | Setback | It can't take reactions until the start of its next turn.
Fouled Follow-Through | Setback | It can't move for the rest of this turn.
Jarred | Setback | If it's concentrating on a spell or effect, it must succeed on a DC 10 Constitution saving throw or lose concentration.
Twisted Limb | Setback | Its movement provokes one additional opportunity attack this round.
Shield Knocked Aside | Setback | If it uses a shield, it loses the AC bonus until it spends a bonus action righting it.
Numb Limb | Setback | Its next attack roll with that limb is made with disadvantage.
Winded | Setback | On its next turn it can move or take an action, not both. Bonus actions still work.
Weapon Caught | Setback | Its weapon lodges in armour, a shield, or timber. Freeing it takes an action and a DC 10 Strength check.
Poor Footing | Setback | It has disadvantage on Dexterity saving throws until the end of its next turn.
Read by the Party | Setback | The next creature to make a saving throw against one of its abilities has advantage on that save.
Struck Itself | Setback | It takes 1 damage and looks thoroughly ridiculous doing it.
Loosened Barding | Setback | It has disadvantage on the next Strength check or Strength saving throw it makes.
Momentum Lost | Setback | It can't make any additional attacks this turn, even if it had them remaining.
Fouled an Ally | Setback | An allied creature within 5 feet is put off. Its next attack roll is made with disadvantage.
Down to One Knee | Setback | It isn't prone, but it's low and awkward. Standing fully costs 5 feet of movement.
Weapon Dropped | Blunder | Its weapon falls into its space. Picking it up costs an object interaction.
Weapon Flung | Blunder | The weapon sails 10 feet in a random direction.
Overbalanced | Blunder | It falls prone.
Full Whiff | Blunder | The attack misses so completely that its turn ends immediately.
Struck the Terrain | Blunder | The weapon bites deep into stone or timber. It takes 1 damage, and freeing the weapon takes an action and a DC 12 Strength check.
Carried Away | Blunder | Its own momentum moves it 5 feet in the direction of the swing, DM's choice of space.
Legs Give Out | Blunder | Its speed is 0 until the end of its next turn.
Concentration Broken | Blunder | It loses concentration on any spell or effect it's maintaining. No saving throw.
Knocked an Ally Aside | Blunder | An allied creature within 5 feet is shoved 5 feet and knocked prone.
Caught by the Terrain | Blunder | Rubble, roots, or a corpse snares it. It's restrained until it uses an action to break free with a DC 12 Athletics check.
Backswing | Blunder | It takes damage equal to one roll of its weapon's damage die, with no modifiers.
Blinded by Its Own Gear | Blunder | Helm, mane, or hood comes down over its eyes. It's blinded until it uses an object interaction to clear them.
Harness Sundered | Blunder | A piece of its visible gear tears loose and falls into its space.
Wild Arc | Blunder | The creature nearest to it — ally or enemy — must succeed on a DC 10 Dexterity saving throw or take 1 damage.
Exposed Flank | Blunder | The target may immediately make one opportunity attack against it.
Gasping | Blunder | It has disadvantage on all attack rolls until the end of its next turn.
Shield Ripped Away | Blunder | If it uses a shield, the shield is torn loose and lands 10 feet away.
Lost the Line | Blunder | It's carried 10 feet away from its target by its own swing.
Weapon Bent | Blunder | The weapon deals 2 less damage (minimum 1) for the rest of the encounter.
Crashing Fall | Blunder | It falls prone and drops whatever it was holding.
Deep Self-Wound | Disaster | It takes damage equal to one roll of its weapon's damage die plus its ability modifier.
Hamstrung Itself | Disaster | Its speed is halved for the rest of the encounter.
Struck an Ally | Disaster | An allied creature within 5 feet takes one roll of its weapon's damage die, with no modifiers.
Limb Wrenched | Disaster | It has disadvantage on all attack rolls made with that limb for the rest of the encounter.
Weapon Lost | Disaster | The weapon is flung 20 feet in a random direction, into whatever the terrain offers.
Stunned by Its Own Blow | Disaster | It's stunned until the end of its next turn.
Armour Split | Disaster | Its AC is reduced by 2 for the rest of the encounter.
Footing Collapsed | Disaster | The ground gives beneath it. It falls prone and is restrained until it uses an action to climb out.
Blood in Its Eyes | Disaster | It's blinded until the end of its next turn.
Enraged and Reckless | Disaster | It abandons all defence. It has advantage on its attack rolls until the end of its next turn, and attack rolls against it have advantage for the same duration.
Weapon Broken | Disaster | A non-magical weapon is ruined. A magical one is badly bent and deals 2 less damage (minimum 1) for the rest of the encounter.
Pulled Into Reach | Disaster | It stumbles into the target's space and provokes an opportunity attack from every hostile creature that can reach it.
Ribs Cracked | Disaster | It has disadvantage on Constitution saving throws for the rest of the encounter.
Broke Its Own Formation | Disaster | Every allied creature within 5 feet must succeed on a DC 12 Dexterity saving throw or fall prone.
Snuffed Its Own Light | Disaster | Any light source the creature carries or produces goes out. If it relies on darkvision, so much the better for it — and worse for everyone else.
Winded and Down | Disaster | It falls prone, and its speed is 0 until the end of its next turn.
Dropped Its Prize | Disaster | Something the creature was carrying — DM's choice, and it should be something the party wants — falls into its space, in reach of anyone bold enough.
Nerve Struck | Disaster | It drops everything it's holding and can't pick anything up until the end of its next turn.
Deafened | Disaster | It's deafened for 1 minute, and can no longer hear its allies' warnings.
Momentum Betrayal | Disaster | It takes one roll of its weapon's damage die and falls prone.
Ally Struck Hard | Catastrophe | An allied creature within 5 feet takes the attack's full damage, modifiers included, exactly as though it had been hit.
Weapon Shattered | Catastrophe | A non-magical weapon is destroyed outright. A magical one is unusable for the rest of the encounter.
Impaled Itself | Catastrophe | It takes double its weapon's damage die and is badly wounded: disadvantage on attack rolls until it regains any hit points.
Thrown Clear | Catastrophe | It's hurled 15 feet in a random direction, lands prone, and takes 2d6 bludgeoning damage.
Blinded | Catastrophe | It's blinded for the rest of the encounter.
Limb Broken | Catastrophe | The limb is ruined — permanently, barring magical healing. It loses any attack or ability that requires it.
Structural Collapse | Catastrophe | Its swing brings the room down on itself. Every creature within 10 feet must succeed on a DC 13 Dexterity saving throw or take 2d6 bludgeoning damage, and the DM may collapse a piece of cover permanently.
Roused the Wrong Thing | Catastrophe | The noise carries somewhere it shouldn't have. Something arrives in 1d4 rounds, and it is not necessarily on the monster's side.
Prisoner Freed | Catastrophe | Anything the creature was grappling, restraining, swallowing, or otherwise holding is released immediately.
Weapon Turned on It | Catastrophe | If the target has a free hand, it may use its reaction to make one attack against the monster using the monster's own weapon.
Weapon in the Party's Hands | Catastrophe | The weapon lands in a player character's space. They may pick it up as a free object interaction, and it is exactly as good as it looked.
Concussed | Catastrophe | It's stunned until the end of its next turn, and has disadvantage on all ability checks for the rest of the encounter.
Friendly Fire, Badly | Catastrophe | Make the attack roll again against the allied creature nearest to it, using the same modifiers. On a hit, that ally takes full damage.
Everything Falls | Catastrophe | It drops its weapon, its shield, and one carried item, each into a different space within 10 feet.
Out of the Fight | Catastrophe | It falls prone, drops its weapon, and its turn ends. It can't take reactions until the start of its next turn.
Brought the Room Down | Legendary | It strikes something load-bearing. The DM introduces a collapse, a hazard, or a countdown. The creature takes 4d6 bludgeoning damage and is prone and restrained under rubble until it uses an action to dig itself out.
Weapon Destroyed Utterly | Legendary | The weapon is gone — shattered past repair, swallowed by a chasm, or melted to slag. A magical weapon does not survive this either. If the party wanted it, they can't have it now.
Struck Its Own Champion | Legendary | The attack resolves in full against the most dangerous allied creature within its reach, as a critical hit. Roll the damage dice twice.
Knocked Itself Out | Legendary | The creature is unconscious for 1 minute, or until it takes damage. For most encounters this simply ends its participation, and the party has every right to take advantage of it.
The One They'll Tell Stories About | Legendary | It ends up disarmed and prone with its own weapon buried somewhere absurd and conspicuous. Every creature that can see it has advantage on attack rolls against it until the start of its next turn, and each of its allies that witnessed this must succeed on a DC 10 Wisdom saving throw or have disadvantage on its next attack roll. Morale is a real thing, and it just took a serious blow.
Questions
Do critical fumbles exist in 5e?
No. A natural 1 on an attack roll misses, and that is the whole rule. Fumble tables are a house rule, and a popular one — but nothing in the game requires them.
Should a natural 1 always miss?
Yes, on an attack roll — that part IS a rule. A table like this describes the miss; it does not change whether you hit.
Doesn't this punish martial characters?
It can. A fighter with three attacks rolls far more d20s than a wizard, so a per-roll fumble rule falls hardest on the people who attack most. Rolling only on a confirmed fumble evens it out considerably.
